Click the order form image on the left for a printable order form. A new window will appear, Use your browsers Print option under the File pull-down menu to print a copy. Once printed, close the window to return to this page.

Note: To print the order form you should first view it using "Text Size: medium" in Internet Explorer or "Text Size: normal" in Mozilla Firefox. The Text Size option for both browsers is available under the View pull-down menu at the top of your browser's window.

Once the form has printed, please fill it in and send it along with your cheque to the address at the top of the form.
